The EGR liquid cooler on the M15A-FXE can develop internal leaks or crack, allowing coolant to enter the combustion chambers directly. A TSB (EG-00691T-TME) is known.
Symptoms: White smoke from exhaust, dropping coolant level, engine warning light, in worst case coolant in engine oil (milky appearance)
Recall for Yaris Hybrid (MXPH11) built between 07/2020β04/2021: a software fault in the brake actuator can deactivate ABS, the pre-collision system and the parking brake. Approximately 144,500 vehicles affected across Europe.

As with the M15A-FKS, the liquid-cooled EGR cooler can develop cracks leading to coolant ingress into the cylinders. The Atkinson-cycle operation increases thermal stress.

The electric coolant control valve is a documented weak point across all Dynamic Force engines. The M15A-FXE is affected by pan-European EGR cooling issues per TSB EG-00691T-TME.

With short city trips, the hybrid's small 12V auxiliary battery does not recharge sufficiently and discharges. This is one of the most common breakdown causes on the Yaris Hybrid XP21.
Symptoms: Vehicle will not start, display remains dark, 12V system warning
General oil loss at sealing surfaces of the M15A-FXE is occasionally reported. The complex architecture with eight injectors and a high-pressure pump makes diagnosis and repair labour-intensive.
