The 6.0 TDI V12 carries two turbochargers in a cramped engine bay. Thermal stress and oil supply problems cause turbocharger damage; replacing both units costs 5,000–12,000 €.
Symptoms: Massive power loss, blue smoke, turbo whistling or whining, engine oil loss
The V12 has twelve piezo injectors, all of which must be replaced when worn. Even a partial replacement costs many times more than on smaller engines.

The densely packed engine bay of the Q7/A8 with the V12 creates extreme heat pockets. The cylinder head gaskets on the rearmost cylinders (near the firewall) are particularly at risk; repair is extremely laborious.
Symptoms: Overheating warning, white smoke from the exhaust, coolant loss, oil foam in the coolant reservoir
The V12 timing chain system with six camshafts and multiple chains is extremely complex. Maintenance and repair costs far exceed those of a normal V6; failures threaten from 200,000 km.

12-cylinder architecture requires an elaborate timing chain drive with multiple chains for both cylinder banks, camshafts, and high-pressure pump. Maintenance is extremely expensive.

6.0 V12 TDI generates enormous heat in urban driving (up to 20 l/100 km at full load). Cooling system components (fans, radiator) show premature wear from sustained thermal stress.

Air bags become porous, the WABCO compressor overheats and fails. The rear axle typically drops first. Full repairs regularly cost 2,000–3,500 €.
The 3.0 TDI timing chain sits on the gearbox side and can elongate from 120,000 km. Engine and gearbox must be separated — cost 3,500–8,000 €. Chain jumping means total engine damage.
The vehicle weight of over 2.3 tonnes leads to heavily elevated brake wear. Pad wear as early as 22,000 km in urban traffic documented. Front disc and pad change costs up to 1,600 €.
The panoramic roof drainage channels block with leaves and dirt. Water runs down the A-pillar to the fuse box and can damage electronics.
The keyless entry receiver antennas sit in the door handles and fail due to moisture and thermal expansion. Vehicle can only be opened with mechanical key.
